    Individualized meal plan
    How to fit in daily bread and potatoes
    How to fit in occasional "sweets"
    Grocery store tour/dining out (with a registered dietition)
  • Fears and feelings
  • Complications
    Retinopathy (eye damage)
    Nephropathy (kidney damage)
    Neuropathy (nerve damage)
    Amputations
  • Emergencies/Sick-day Management
    Hypoglycemia (low blood sugar)
    Hyperglycemia (high blood sugar)
  • Exercise
  • Travel
  • Foot Care/Good Hygiene
  • Health Care/Community Resources
  • Total Physical Assessment
  • Blood glucose monitoring
  • Long-term follow-up

Meet with the Diabetes Education Team, Cassandra B. Ford, RPh, CDE, Laura "Kim" Yokum, RD, and Shari Thompson, RN.

Monday, Tuesday, Wednesday - 8:00am-4:00pm or by appointment. Fees are billed to your insurance, Medicare or Medicaid.

  • 152 people will lose a limb
  • 76 people will start kidney dialysis
  • 49 people will go blind
