Hospice Is...
...a program designed to meet the needs of persons with a terminal illness to allow them to live as comfortably and fully as possible until death occurs.

...a Home Care service designed to provide the support needed to enable the patient to remain in familiar surroundings with the family and friends rather than in a hospital or other facility.

...care provided by a team of doctors, nurses, social workers, nursing assistants, therapists, counselors, pastors and trained volunteers. The Hospice team works with the patient and the caregivers to manage pain and other symptoms caused by the disease as well as providing spiritual, psychosocial and physical care that is needed.

...paid for by Medicare, Medicaid and many private insurance companies. Medicare and Medicaid cover all staff visits, all necessary durable medical equipment (such as hospital beds), and the cost of medications used to manage the terminal illness. Up to five (5) days of inpatient respite care may also be available. All billing forms and paper work will be completed by the Hospice staff.

...bereavement support for the caregivers for one year after the death of the patient.


The Hospital Team Members Are...
...the patient's Physician. The Hospice Medical Director is also available for consultation.

...the Nursing Staff. Each Hospice patient's care is coordinated by a registered nurse who will visit the patient's home to provide care. A registered nurse is on-call 24 hours a day, seven (7) days a week, to assist with problems or to answer questions.

...the Social Worker. The social worker provides help with any emotional or financial difficulties.

...Hospice Aides. Certified home care aides provide personal care such as assisting with a bath or shampoo.

...trained Volunteers who provide support in many areas depending on the needs of the patient and family.

...the Clergy. The patient's own pastor will provide spiritual support. If needed, the Hospice will arrange for visits by a spiritual counselor.

..Therapists and Counselors when needed.


Hospice Services...
...are available for Grant, Hardy, and Pendleton County residents who have a terminal illness and only need treatment for comfort measures rather than for the cure of the disease.

...focus on the management of pain and other discomforts to allow a better quality of life even though the disease cannot be cured.

...are provided without discrimination on the grounds of race, color, national origin, sex, religion, disability, age or nature of the illness.


Referrals...
...are accepted from anyone: friends, family, health care professionals.
