
In scheduling appointments, we do our best to accommodate your busy schedule when reserving your appointment times. We will work with you to the best of our capabilities to establish a mutually agreed upon schedule for your rehabilitation. We realize your time is very valuable and we work very hard to stay on schedule. It is also important for you to be timely with arriving for your appointment time and allowing yourself the entire treatment time that your therapist has scheduled for you. We require advance notice of appointment cancellations or rescheduling.
New patients should arrive at least 20 minutes prior to your scheduled appointment time to allow for the registration process and completion of required forms. Your initial evaluation is scheduled for a one hour time period to allow for a thorough evaluation by the therapist. Your first visit will typically be longer than your following visits.
At your initial appointment your therapist will get a detailed history from you and review all necessary medical information. You will then receive a complete evaluation and assessment of the problem that you are being referred for. Your therapist will design treatment goals and a plan to help you meet your goals. You will receive education on your diagnoses, a home exercise program and answers to any question you might have. Treatment frequency and duration will be determined by your therapist.
Bring the following with you to your first visit:
- New Patient History Form completed
- Therapy referral or prescription from your referring physician- required
- Insurance cards
- Necessary medical information (x-ray, MRI or other radiology reports, nerve test, etc.)
- Comfortable clothing for access to the involved areas/joints
